Welcome to the home page for the online forms required by the UIL and Crowley ISD. These forms are required each year before your son/daughter may participate in athletics for Crowley ISD. The athletic department would like to thank you and your son/daughter for filling out the forms and submitting them to us in a timely manner.

To get started, you will need your son/daughter’s Crowley ID number, health insurance card and primary care physician contact.
Note: If you are new to our school district and do not have an ID number assigned you will need to contact your school athletic trainer or coordinator to be able to process your forms. If your son/daughter's ID number starts with a zero, please omit the zero when filling out these forms.

Directions:

1. The required annual Medical History & Physical forms must be submitted prior to participation. To access a blank copy of the Physical/Medical History form, please click the Download and Print tab on the right hand side of the online forms page. Submit the completed medical history and physical forms by uploading documents online or contacting your campus coordinator indicated below.

2. All other required paperwork will be done electronically. Parent/Guardian will be asked to provide information such as home phone numbers, cell phone numbers, email addresses, name of primary care physician, insurance policy information and other pertinent information.

4. To access the required athletic participation forms for Crowley ISD, hold your cursor over the “Electronic Participation Forms” tab. Click on the form name from the drop down list of required forms. If you do not know the answer to a question, please answer the question with N/A, or none. Please select the correct school year and grade for your son/daughter. Use legal names on all forms, no nicknames.

6. To electronically sign the document, click inside the signature box and hold your mouse down, this will allow you to create an “Electronic Signature.” If you make a mistake and need to start over, click on the refresh icon next to the signature box. If you are using an iPAD, you may create an electronic signature, by holding your finger to the screen and signing your name or by using a stylus.

7. Once you have filled out all of the information on each page you will have the opportunity to print the document for your own records. Your school will not need a copy as it has already been submitted online.

8. You will receive a confirmation email once the document(s) have been reviewed by Crowley ISD.
